CareTracker in MR/DD facilities Individualized program plans or service plans can be created for residents so caregivers can document goals, and other information about the resident throughout the shift. CareTracker guides the staff through what’s needed each shift so that everything is completed and documented according to the plan. CareTracker is useful for ICF/MR facilities, Day Hab environments, and Group Homes.
