Advantages of storing detailed data For example, for one resident over the seven day ADL lookback period your nurse aides make 462 observations. 11 ADLs X 2 questions each 22 Questions X 21 shifts in 7 days 462 observations CareTracker stores all 462 observations and computes the 22 answers needed for the MDS (Section G). Most clinical systems don’t have a place to store the 462 detailed observations. Instead, they only store the summarized responses (once they are manually summarized from paper forms). With CareTracker, you have the detailed records that can be used for care planning, trend analysis, quality assurance, and management purposes. For quality of care purposes, the detailed observations provide the much needed information for a precise picture of the resident. One facility had a resident that was wandering quite often. The staff spent 20-30 minutes each day redirecting the resident, time that could have been spent elsewhere. Section E of the MDS indicated that he wandered more than seven times in the last 30 days which is the level of detail maintained by most MDS systems. Once this facility implemented CareTracker, the information going to the MDS was the same. However, by seeing the detailed observations maintained by the CareTracker system, the facility could see that there was one time of day that the resident was wandering more frequently. The facility implemented an activity for the resident at that time each day which saved the time for the staff and reduced the frustration for the resident. With only the summary information, this would be impossible. CareTracker maintains the details and treats the MDS sections as a byproduct of the detailed data.
